Please Note: All patients must have a referral from a licensed medical provider in order to be eligible for physical therapy services. Your provider may be a physician, physician's assistant, nurse practitioner or a dentist. Please bring your referral with you on the day of your initial evaluation.

Please Note: The rate per session decreases when purchasing larger session packages.

Step 3: Schedule Your Session(s)

After purchasing your session package you'll be automatically redirected to our purchase confirmation page where you'll be able to select a doctor of physical therapy and schedule your session(s) on their online calendar. You'll be given full access to this calendar system so you can periodically return to schedule your physical therapy sessions as needed.
